Once you have reviewed the instructions to proceed to online forms
We would like to thank you for participating in the online submission process for athletic participation paperwork. This automated system allows us to process forms more efficiently while conserving valuable resources.

All required forms may be submitted online for review.

* To begin, select “Proceed to Online Forms” below.
* You will be prompted to either create a Parent Account or continue as a Guest to complete the forms.
     - It is encouraged that you create a parent account to have easy access to your student’s records
     - Click HERE for instructions on how to create a Parent Account.
* You may be asked to provide your student’s ID number, date of birth, and other demographic information.
* Use your student’s legal name as it appears on their official school records.
     - Do not use nicknames.
     - If the student has two surnames, include hyphens or spaces exactly as registered with the school.

* For AISD/UIL forms, you must open each document and check the box confirming that you have read and understand the information provided. Both the parent/guardian and student-athlete must provide electronic signatures. NO NEED TO PRINT.
* To sign, click inside the signature box and hold the mouse button down to create an electronic signature. If an error is made, use the refresh icon next to the signature box to restart.
* You will have the option to print the documents for your records. Do not submit printed copies to the school.
